This tab allows the recording of patient assessment results.   This information should be entered following the assessment.  Any assessment is related to an action day.   This information will be allowed to be printed from the patient’s assessment report.

Agent no :  serial number associated to the agent in charge of assessment evaluation.

By pressing the   you may pick and choose from pre-entered agents.

By pressing the Add New Agent icon   you may directly add a new agent.

Assessment date:  assessment date for that patient. The assessment date is, by default, the action day date.

Assessment and Plan: any useful information relevant to the assessment.  Free text format.

The Assessment field provides a full text memo field on which to enter any additional information or remarks that you wish to include in the patient’s record. This page enables you, for example, to enter summary information for the referring physician or follow-up recommendations.

Unlike most word processors, words do not automatically wrap to the next line, so end each line by pressing the Enter key. Pressing the Backspace key deletes the character to the left of the cursor; pressing the Delete key deletes the character under the cursor.
